Patient Engagement and Experience Team at Princess Alexandra Hospital Harlow are developing this with Family Mosaic as they think support for carers is a gap. It has begun as an in-reach programme to wards. Core of the work is about addressing unmet social needs (debt, homelessness, long term mental health, economic inactivity) at ward level, via productive integration with voluntary sector services with the objective of reducing readmissions / improving outcomes in the longer term for patients, but particularly targeting more vulnerable groups, such as older people, people with disabilities, life limiting conditions or those who have capacity but no external advocacy.
