Part of the NHS Five Year Forward View will allow local health services to take decisive steps to secure high quality, joined-up, improved care. Three main areas that may involve service changes are: * Faster progress towards joined up health and social care based around localities, *More collaboration and shared services across the three main hospitals in Basildon, Chelmsford and Southend, *Changes in urgent and emergency care in line with national recommendations. Other areas involved: simplifying commissioning and contractual arrangements, developing the healthcare workforce and improving IT and access to shared care records. The intention is to have clinically sustainable services in financial balance by 2018/19.
Public workshops planned for Sept 2016 to start discussions with local people about services and garner views on potential change, to share people’s experiences and give feedback on what might/might not work in future. Dates will be published here when we have then.

Drop-In Service at Princess Alexandra Hospital, Harlow. Free Service to help address unmet social needs for vulnerable groups – covering advice on debt, benefits, housing available in the hospital via Family Mosaic Weds 10am-12 noon. The hospital trust is working on offering other advice too (i.e.for family carers) and at ward level, via the voluntary sector and the C.A.B. with objective to reduce readmissions and improve outcomes for patients.

NICE Guidelines. As Caretobedifferent.co.uk remind us “NICE guidelines on pressure sore prevention and management could be useful in your Continuing Healthcare case”. Guidance states: care interventions MUST be documented. Check your care provider is recording relevant info in care plan/daily care notes. (NICE: National Institute for Clinical Excellence – Non-Departmental Public Body accountable to Dept of Health – develop guidance/quality standards NHS and Social Care)

Carers’ Passports in hospitals. let us know if you have used one of these and how you got on. For example in Broomfield Hospital Chelmsford a plastic card is available to carers of patients with dementia to access wards more readdily and staff to know who you are.
